What is Support Coordination?
A support coordinator assists individuals to understand, navigate and access services under the NDIS to achieve their goals. Their role is about empowering people to make the most of the available support and live their best lives.
To do this, support coordinators work closely with the participant and their broader support team to understand their needs and develop a personalised plan.Support coordinators then assist in connecting the participants with the right providers and resources.
Once developed and implemented, support coodinators will continue to oversight & involvement to ensure things run smoothly along the way.

Support Coordinator -Gwyn Pieper
With 4 years of experience in the NDIS sector, Gwyn’s roles & responsibilities to date have included directly supporting participants with community access, personal care and SIL, mental health and capacity building group activities.
Having now found her passion as a support coordinator Gwyn has an ummatched understanding of the support services community and potental activities available for participants in the Hunter Vallety. This enables Gwyn to be well positioned in supporting her participants with planning, goal setting, implementing and tracking of participant plans while maximising the impact of their funding on their quality of life.
Gwyn also has a wealth of knowledge and experience in supporting new participants gaining access to funding under the NDIS and current participants undergoing plan reviews to receive the required funding to best support them in achieving their goals.

Coordination of Supports:
Often the best way to support a participant involves multiple service providers, therefore we ensure seamless coordination between various service providers, aiding participants to maintain consistent and effective supports.
Capacity Building:
We work with participants to develop their skills and capabilities, fostering greater independence and confidence in managing their own support needs.
Budget Management:
We provide guidance on managing NDIS funding, helping participants allocate their budgets effectively to maximize the benefits they receive and impact towards meeting their goals and improving their quality of life. In addition to providing assistance with ongoing tracking of spend over the life of each plan.
Plan Review and Adjustment:
We assist participants with regular reviews of their NDIS plans and making necessary adjustments as needed to ensure the plans remain relevant and effective as circumstances change.
Advocacy:
We advocate on behalf of participants to ensure their rights are protected and their voices are heard in decision-making processes.
Crisis Support:
We offer immediate assistance and solutions during emergencies or crises, ensuring participants receive the necessary support when it’s needed most.

We share and stay up-to-date on current NDIS policies, procedures and available resources ensuring both us and participants are always informed and can navigate the NDIS system efficiently and effectively.
